How to Register/Refer a Child

Procedures for Registering a Child with NEC

If you suspect a child is not using his/her vision and hearing or who is "at risk," download the registration packet or Spanish registration packet the and please follow these steps. (Note: Registration packet is in PDF format; please visit adobe.com to download Acrobat Reader if you do not have it installed.)

  1. Complete NEC Initial Information Form and National Deafblind Census Code Sheet.
  2. Obtain signature on Parent Contact Release Form. This form may be in addition to your agency or state form(s). Spanish Contact Release form.
  3. Attach the following reports/documentation:

    • Most recent vision/opthalmological report (or functional vision assessment)
    • Most recent audiological report (or functional hearing assessment)
    • Most recent IEP, IFSP, ITP and other developmental documents
    • Documentation of etiology if available


"You may begin the registration by sending in the Initial Information Form and Code Sheet only. However, registration of a child who is deafblind will only occur after all of the above documentation has been received and reviewed by the New England Consortium of Deafblind Projects."

Fax the forms to NEC at 617-972-7354.
Please make sure your forms are on white paper if you are faxing them in.

Please note: Once a child has been registered and the necessary documentation obtained, NEC will send out the following to child's family and service providers/related agencies:

  • Eligibility Letter/NEC Description
  • Certificate of Eligibility
  • Handouts and/or key articles related to deafblindness

"Initial Child" Referral Process

There is an additional way to register children you are currently working with. Use the Initial Child Referral Form. By completing the categories required. We can temporarily register the child with the national census as "pending" or "at risk" for one year.
