Survey

As part of NEC's mission to provide information dissemination and technical assistance to families, necdbp.org will launch it's annual survey to each state and all the srvice providers that have children registererd witht he center in order to deceide what topics are of the most intereest and need to families and survice providers annually. It is really important that you take a minute and complete an evaluation for each child you have with registered that may recieve services through the center so we can make the best decision appropriate for the community. Below we have the links for each survey, please choose the one that best suits your child's needs.

Survey for the New England Consortium

Complete this Survey to help your child or Spanish Version (Survey in Sp) a child you are working with get the services that best suit that child by completing and returning this survey and release (release is just for parents to complete) to us. If you would like to receive the newsletter or be part of the survey process and are in one of the four states of Massachusetts, Maine, New Hampshire or Connecticut just email us at NEC@Perkins.org to be added to the mailing list. The New England Consortium of Deafblind Projects never releases names or addresses of it's list ever. All mailings you recive through us will be stickly for trainings, a NEC Calendar, our newsletter or announcements such as Alerts from your state that we feel are important to make you aware of.
